IMM 5645 Family Information Form Canada 2027: How to Fill It Section by Section
The Family Information Form (IMM 5645) asks about your spouse or partner, parents, children and siblings. Every applicant aged 18 or older applying from outside Canada for a visitor visa, study permit or work permit must complete it. This guide walks through each section using IRCC’s own instructions.
Family information form Canada 2027: IMM 5645 is IRCC’s “Family Information Form – Visitors, Students and Workers”. It has four parts: Section A (you, your spouse or common-law partner, mother and father), Section B (all your children), Section C (all brothers and sisters, including half- and step-siblings) and Section D (certification). For each person you give the full name, relationship, date of birth, marital status, present address, occupation and whether they will come to Canada with you. Answer every question; write “N/A” if something does not apply.
What is the IMM 5645 Family Information Form?
It is a standard IRCC form that collects details about your immediate family. Visa officers use it alongside your main application, so the information must match everything else you submit, such as your marital status and your invitation letter.
